CHDCA Checklist to obtain certification via mail

1. Submit CHDCA Application
2. Submit copy of original certificate from IKECA or PWNA if applicable
3. Submit proctor application
4. Submit payment for LIFETIME Membership with application of $2500 made payable to: Advanced Marketing, 110 Beverly Dr. Madison, TN 37115
5. Purchase/Study NFPA 96 2008 standards
6. Arrange date/time to take exam with your proctor
7. Take exam and have the proctor mail the exam back to CHDCA
